These Terms do not constitute a therapeutic relationship. A therapeutic relationship is only established through a signed Client Disclosure & Informed Consent Form and an initial intake session.

Alchemy provides individual psychotherapy services on both an in-person and telehealth basis.
3.2 No Therapeutic Relationship via Website
Sending a message through this website, submitting a contact form, or reading content on this site does not create a therapist-client relationship. A formal therapeutic relationship begins only after both parties have signed a Client Disclosure & Informed Consent Form.
